For Referring Physicians:
Thank you for referring your patients to the University of Michigan Department of Physical Medicine & Rehabilitation. To coordinate a patient transfer, consult with another physician or inquire about clinical services at the University of Michigan, contact M-Line at (800) 962-3555.
For appointments only, you also have the option of completing a Consult Request Form for either an adult or pediatric patient and faxing it directly to the clinic.
To refer a patient to our Adult Post-COVID Clinic, please complete this specific Post-COVID Consult Request Form, which includes a fax cover sheet with details on additional required information to fax in with the referral.
- Note: Please review the Evaluation and Treatment Guide for Post-Acute COVID-19 Sequelae, a set of guidelines for initial management of symptoms in the primary care setting, prior to determining whether a referral to our clinic would be appropriate.

Our admissions coordinator will work with case managers, social workers, area hospitals and families to admit patients to our Adult and Pediatric Acute Inpatient Rehabilitation Services.
