My patient care responsibilities include: (1) co-managing diabetes, hypertension and hyperlipidemia with patients’ PCPs and/or specialists and comprehensive medication reviews at the Northville and Livonia Health Centers, (2) facilitating transitions of care (TOC) for patients recently discharged from UM hospital with our PharmD TOC team and (3) supporting the House Calls team, a group that support UM’s home bound patients. My administrative activities include: (1)working to optimize medication use across UM as the managing pharmacist of the Medication Optimization program and (2) acting as a liason between payors, UM and other physician organizations across the state to support projects that better patient care. In my spare time I enjoy spending time with my family, my pets, and time outside in my flower garden.
