Clinical Care Settings

Clinical Care Settings

Fellows will have the opportunity to participate in a diverse variety of rotations across several clinical sites, including primary and consultative care in acute, long-term care, as well as in home care settings. Fellows will work alongside our prestigious faculty and amazing interprofessional teams within geriatric and palliative care. We also offer training in Palliative Medicine as well as specific training for qualified fellows interested in a master’s degree in clinical investigation.

Geriatrics Inpatient Consult Team (GICT)

  • Environment: Michigan Medicine Medical Campus (inpatient wards, emergency department)
  • Goals: As part of a multidisciplinary consult team, provide consultative care recommendations to primary teams caring for older acutely-ill medical, surgical, and psychiatric elderly patients while learning clinical decision-making skills

Hospice

  • Environment: Arbor Hospice, one of the largest non-profit hospice organizations in Southeastern Michigan providing hospice services to patients with advanced and end-stage diseases.
  • Goals: To develop competencies related to the provision of end-of-life care in the community, following both patients receiving care at Arbor Hospice in-patient residential facility and patients receiving home hospice services.

Huron Valley PACE

  • Environment: Huron Valley PACE, is located in Ypsilanti, MI and provides all the care and services covered by Medicare and Medicaid. Serving older adults have ongoing healthcare needs and live within our service area.
  • Goals: To develop competencies in this innovative model of care where participants remain independent in their communities while receiving individualized integrated medical care.

 Outpatient Sub- Specialty Clinics

  • Environment: Turner EAA Geriatric Clinic, one half day per week for two months for each specialty clinic
  • Goals: To increase knowledge of the following diverse specialties: Endocrinology, Mobility, Nephrology, Neurology Cognitive Disorders, Neurology Movement Disorders, Psychiatry, and Rheumatology

Post-Acute Rehabilitation

  • Environment: Care and Rehabilitation Center at Evangelical Home (50-60 patients who require a mix of long-term and sub-acute skilled care)
  • Goals: To effectively manage sub-acute care and skilled nursing home patients, while understanding administrative requirements of this setting, including the role of a medical director

VA Community Living Center

  • Environment: VA Ann Arbor Healthcare System
  • Goals: To be a proficient member of a multidisciplinary team, evaluate the complex problems of elderly patients in the Community Living Center (CLC) and communicate recommendations

 VA Home-Based Primary Care

  • Environment: VA Ann Arbor Healthcare System
  • Goals: To provide care and perform geriatric assessments in home for home-bound seniors with multiple comorbidities

 VA GeroPsych

  • Environment: VA Ann Arbor Healthcare System 
  • Goals: To provide consultative evaluation and management services to older medical and surgical patients in the context of clinical situations requiring psychiatric input such as chronic pain, determination of decisional capacity, physical rehabilitation, chronic illness, somatization, delirium, dementia, and psychosis

 VA Palliative Care

  • Environment: VA Ann Arbor Healthcare System 
  • Goals: To provide palliative care and support to terminally ill patients and their families, understand hospice resources and increase knowledge of both pain and symptom management
