What to Bring to Your Appointment

If you are being seen for fertility questions, or possible fertility treatments, you should receive an intake questionnaire. If you have not received this within one week of your appointment date, please call 734-763-6295, option 1 to confirm it has been sent. The intake questionnaire includes questions for the individual who has made the appointment. The final page of the questionnaire includes questions for a male partner, if applicable. Please fill the form out as completely as possible.

If you have had fertility treatment or evaluation elsewhere, please bring your records with you. This would include, but not be limited to, documentation of response to prior fertility medication cycles, documentation of prior IVF cycles, lab tests such as FSH, AMH, TSH, and prolactin, prior semen analysis results, hysterosalpingogram results, and ultrasound reports.

If you are not being seen for fertility questions or possible fertility treatments, please bring any records with you that you think would be pertinent to your care.
